PLS Egg Donation

How old are you?

PLS egg donors are 21 to 30.

Which state do you live in?
What are your height and weight?

We use these to work out your BMI. We keep the BMI, not your height and weight.

Are you a non-smoker who uses no illegal drugs?
Do you have a high school diploma?

A GED counts. Further education is welcome but not required.

Do you have a valid Social Security number?

Answer yes or no only. We never ask for the number here.

Are you open to a psychological evaluation and medical screening?

Both are paid for by the intended parents.

Last step: how do we reach you?
Have you had a healthy pregnancy? (optional; preferred, not required)
