What does the Private Label Surrogacy program fee cover?

The short answer

The Private Label Surrogacy program fee covers everything the agency itself does: consultation, recruiting and seven-step surrogate screening including physician record review by Mark Silverberg, MD, matching run personally by Nancy Block, RN, coordination of your clinic, independent attorneys, escrow agent and insurance review, case management by a former surrogate through birth, nutrition and mental-health support for the surrogate, birth plan and hospital coordination, and rematch if a match ends before transfer. The program fee is $47,500. It is paid $22,500 [ESTIMATE] at signing, $20,000 [ESTIMATE] at surrogate acceptance and $5,000 [ESTIMATE] at medical clearance. The Assured tier, at $62,500 [ESTIMATE], adds priority matching, unlimited rematches and no new agency fee if the first surrogate does not succeed.

The program fee is set at $47,500. The installment amounts, the Assured tier and the egg donor match fee are marked for confirmation until Nancy Block, RN, confirms them. Your agency agreement is the final word.

What services are included in the program fee?

The program fee covers everything Private Label Surrogacy itself does, from your first consultation to the birth:

What is not in the list is as deliberate as what is. PLS does not employ attorneys, psychologists or escrow agents, and it does not hold client money. Those services are independent so that the people who protect you, and the people who protect the surrogate, answer only to their clients.

What is the payment schedule and what triggers each installment?

The program fee is $47,500. It is paid in three installments: $22,500 at signing, $20,000 at surrogate acceptance and $5,000 at medical clearance.

Each installment follows completed work:

  1. Signing. You sign the agency agreement. PLS opens your file, begins coordinating with your clinic, and starts matching.
  2. Surrogate acceptance. You and a surrogate accept the match. The attorneys begin the gestational carrier agreement, and escrow is funded with $10,000.
  3. Medical clearance. Your clinic clears the surrogate medically. Screening, matching and contract coordination are complete, and the cycle can start.

An installment whose trigger never occurs is not owed.

Tier Total Installments and triggers Rematch terms
Standard $47,500 $22,500 signing / $20,000 surrogate acceptance / $5,000 medical clearance Rematch at no new program fee if a match ends before transfer
Assured $62,500 [installments to confirm] Priority matching, unlimited rematches, no new agency fee if the first surrogate does not succeed
Sibling journey $23,750, half the standard fee [installments to confirm] [to confirm]
Cycle Management (your own surrogate) $15,000; $12,500 for active-duty military [installments to confirm] Not applicable: you bring your surrogate
Egg donor match $11,000; $8,500 with a PLS surrogacy journey [installments to confirm] [to confirm]

What is the difference between the Standard and Assured tiers?

The Standard tier is the full PLS program. It includes rematch at no new program fee if a match ends before embryo transfer.

The Assured tier, at $62,500, adds three things: priority matching, unlimited rematches, and no new agency fee if your first surrogate does not succeed.

What "does not succeed" means for the Assured tier (no pregnancy, or no live birth) is being confirmed.

The Assured tier fixes the agency-fee risk. It does not fix clinic costs or surrogate compensation, which no agency controls. It suits families who expect to need more than one transfer, or who want the agency fee settled once regardless of how the first match goes. See fixed-price versus variable programs for how the tiers compare with guaranteed programs.

What is not included and paid to third parties?

The program fee never includes:

Intended parents typically budget $150,000 to $210,000 all in, including clinic fees and insurance. An itemized PLS journey runs about $131,000 to $155,000 before clinic fees and any new insurance. See the agency fee explained and the full cost of surrogacy.

What do sibling journeys, cycle management and egg donor matching cost?

Sibling journeys. Families returning to PLS for a sibling pay $23,750, half the standard program fee. Your file and your clinic relationship are already in place, so the fee reflects that. See sibling journeys.

Cycle Management. If you bring your own surrogate, such as a friend or relative, Cycle Management is $15,000, or $12,500 for active-duty military. PLS coordinates the clinic, attorneys, escrow, insurance review and case management, without recruiting or matching (scope to confirm). You still need independent attorneys for both sides and independent escrow, because the protections matter just as much when the surrogate is someone you know. See cycle management with your own surrogate.

Egg donor matching. The egg donor match fee is $11,000, or $8,500 when combined with a PLS surrogacy journey. Donor compensation and the clinic's donor cycle fees are separate.

Where does each payment go?

Program fee installments go to PLS. That is the only money PLS receives.

Everything else goes somewhere you can verify:

PLS has used independent licensed escrow since it opened in 2017. When the Surro Connections agency collapsed in late 2025, and when the SEAM escrow company failed with more than $16 million involved, the families exposed were those whose money sat with the agency or a captive escrow. Separating the program fee from everything else is how PLS keeps your money out of that risk.

Before you sign any agency agreement, PLS's included, check these points in the document itself:

  1. The total fee and each installment, with the exact event that triggers it.
  2. What happens to unpaid installments if you stop before their triggers occur.
  3. What a rematch costs before transfer and after transfer, and how many rematches are covered.
  4. Which services are listed as included, and whether any "administration" or "coordination" fees sit outside the program fee.
  5. A statement that the agency does not hold client funds, and the name of the escrow agent.
  6. Whether the agency receives referral fees from any clinic, attorney or escrow company.

If a point is not in writing, ask for it to be added. Nancy's team will walk through every line with you at the consultation, and your own attorney should read the agreement before you sign it.
